Prognostication - If Only Someone Had Warned Us

This course will discuss the importance of regaining the professional skills of evidence-based prognostication. It will discuss the risks of treatment associated with geriatric syndromes and why we should be aware of these diagnoses when we recommend care. We will also touch on the important issue of understanding the available medical evidence for managing the advanced elderly.

Daniel R. Hoefer, MD

Chief Medical Officer, Sharp HealthCare’s Outpatient Palliative Care Program, Transitions & Associate Medical Director, Sharp HospiceCare

Dr. Daniel Hoefer is the Chief Medical Officer for Sharp HealthCare’s outpatient palliative care program, Transitions and Associate Medical Director for Sharp HospiceCare. In addition, he is a board certified family practice physician, board certified in Palliative Medicine and is part of Sharp Rees Stealy Medical Group in San Diego. Sharp’s Community Based Palliative Care Program, Transitions, has won several awards including the Circle of Life award from the American Hospital Association, the National Association of ACOs Innovators award, the Joint Commission Certificate of Distinction and in 2019, the Hearst Health Jefferson School of Population Health Award.

His background includes participation on the Risk Management and Utilization Review Committees for Sharp Rees‐Stealy since 1994. He has spent over 25 years working in nursing homes caring for ventilator dependent and geriatric patients. Dr. Hoefer is EPEC trained, a leading visionary for the development of an evidence‐based care model for late stage/advanced illness and is a leading national and international speaker. Dr. Hoefer was a presenter at the World Congress of Geriatrics and Gerontology in Kaohsiung, Taiwan in November 2015 and has presented to the Department of Justice on several occasions. Additionally, Dr. Hoefer has coauthored research articles published in medical journals and was an adviser to the California Department of HealthCare Services. In September 2016, Dr. Hoefer was the recipient of the first ever Doris A. Howell Award for Advancing Palliative Care.

Dr. Hoefer received his undergraduate degree from UCLA and his medical degree from Eastern Virginia Medical School. He completed his residency at Northridge Family Practice.
