A Picture is Worth a Thousand Words: Using Communication to Enhance Care Transitions

Effectively managing transitions of care is a crucial, challenging part of palliative care and hospice practice. Transitions extend beyond geography to include the evolution of care goals and preferences, hand-offs among medical providers, and adjustments to the caregiving structure. Facilitating these transitions requires both an understanding of what the future may hold, as well as an appreciation for past and present experiences.

In this session, we will explore how effective communication enables the palliative care and hospice provider to work collaboratively with patients and families to “paint the picture” that reflects optimal quality of life in the setting of serious illness.
